Everything you need to prepare for your official Licensure Examination application.
Essential Documents
- PSA Birth Certificate (Original & Photocopy)
- PSA Marriage Contract (For Married Females)
- Transcript of Records (TOR) with Scanned Picture & “For Board Exam Purposes”
- Valid Government ID (Passport, UMID, PhilID, etc.)
- NBI Clearance (For certain boards)
Application Steps

Online Account Creation
Register at the official LERIS portal and upload a professional 2×2 ID photo with a white background.

Select Transaction
Choose “Examination” and select your specific board, date, and preferred regional office.

Payment of Fees
Pay via GCash, Maya, or Credit Card. Keep a digital copy of your payment confirmation.

Appointment & Submission
Visit the PRC office on your scheduled date to submit your physical documents for verification.
Always double-check your TOR. Ensure it has the “For Board Exam Purposes” remark, or the PRC will not accept your application.
