Notarize Affidavit of Loss
Visit a Notary Public. State the circumstances of the loss (where, when, and how). You will need this for your PRC appointment.

LERIS Appointment
Log in to LERIS. Select “DUPLICATE” as the transaction type. Choose your branch and pay the fee online.

On-Site Submission
On your scheduled date, go to the PRC window with your printed application form and notarized affidavit.
Checklist of Requirements
- Notarized Affidavit of Loss (Original)
- Printed LERIS Duplicate Application Form
- One (1) Valid Government-Issued ID
- 1 Passport-size photo (White background with nametag)
- Official Receipt (E-Receipt from LERIS)
Statutory Replacement Fee
₱300.00 – ₱450.00
Note: If you are also replacing a lost Certificate of Registration (COR), the fee is ₱300.00 and requires a separate Documentary Stamp (₱25.00-₱40.00).
